Cool Kids Anxiety Program: Expression of Interest

I acknowledge the information I provide will be used by Logic Lounge Psychology to review whether the Cool Kids Anxiety Program is suitable for my child. I acknowledge that if deemed suitable, my child and I will be offered a formal assessment session to determine suitability of the program for my child’s needs.

By participating in Cool Kids Anxiety Program, I acknowledge that the information that I have entered may be used for supervision, quality assurance, research or training. I may also be invited to participate in additional related research activities. I will have the right to decline any research invitation without jeopardising the completion of the program.
