This document is a guide outlining the essential information needed for a smooth client onboarding experience, ensuring that all relevant details are collected efficiently for a seamless transition into our services.
Client Information
When onboarding a new client, please gather the following information:
- Full Name: The client’s complete name.
- Date of Birth: The client’s date of birth.
- Email Address: The client’s email for communication.
- Home Address: The client’s residential address.
- Mobile Number: The client’s contact number.
- Reference number: The number to claim for the session
Cancellation Policy
We have a cancellation policy that requires 24 hours’ notice for any changes or cancellations. Please ensure clients are aware of this policy to avoid any misunderstandings.
Notes:
After onboarding the client, please leave a note indicating that you have completed the process. For example, you can write: “15/5 JB onboarded. VS and notes.” Additionally, remember to send the intake form to the client for their records.
Client Prerequisites: Approved VS claim with their VS number [00-xxxxx]
- Session Entitlement: 22 sessions per claim. Client can have multiple claims.
- Approval Process: 5-day approval process, approved via VS.
- Intake Form: 18+/under 18
Booking process
- 5-day approval process, approved via VS.
- Must be added to VS portal – contact Janice to add sessions into portal
- Book in 6 sessions minimum
