Essential Client Information
When onboarding a new client, please gather the following information:
- Full Name: The client’s complete name.
- Date of Birth: The client’s date of birth.
- Email Address: The client’s email for communication.
- Home Address: The client’s residential address.
- Mobile Number: The client’s contact number.
- NDIS Plan Number: The number to claim for the session
- NDIS Plan – Start and End Date
- Is the Plan – NDIA, Self-Managed or Plan Managed
- NDIS Goals
- NDIS Service Agreement – Signed copy
Cancellation Policy
We have a cancellation policy that requires 7 days notice for any changes or cancellations. Please ensure clients are aware of this policy to avoid any misunderstandings.
Documentation and Follow-Up
After onboarding the client, please leave a note indicating that you have completed the process. For example, you can write: “15/5 JB onboarded. Medicare and notes.” Additionally, remember to send the intake form to the client for their records.
