Workcover Checklist for Onboarding New Clients
This document is a guide outlining the essential information needed for a smooth client onboarding experience, ensuring that all relevant details are collected efficiently for a seamless transition into our services.
Client Information
When onboarding a new client, please gather the following information:
- Full Name: The client’s complete name.
- Date of Birth: The client’s date of birth.
- Email Address: The client’s email for communication.
- Home Address: The client’s residential address.
- Mobile Number: The client’s contact number.
- Complete the following:

- Claim number – Ask for their claim number (Does not always have the same format)
- Insurance Organisation – Ask who their insurance is, once they tell you, put the insurance as the clients organization
- Case Manager Contact Details – name, email & phone number
- Add Referral
- Date
- Exp date
- Referral Type: WorkCover/CTP
- Limit: # of approved appointments
- Referrer: GP name
- Add Files
- GP Referral – template “GP Referral 16092025 A. Nasibi”
- Treatment Approval – template “WC Approval 16/09/2025”
- Comments – normally on page 3 of treatment approval
